Overview

Nexcentric was engaged to build a centralised platform for care service providers managing high-volume client data, complex rostering and heavy administrative workloads.

The result — ClientCenter — is an AI-enhanced CRM and operations hub that removes manual data entry, files communications automatically, and lets staff interrogate their own data by asking a question rather than building a report.

The challenge

Care providers routinely run on data spread across spreadsheets, individual inboxes and SMS threads on personal phones. That fragmentation produces four predictable problems:

  • Administrative overload — staff spending hours manually copying case notes out of emails and texts
  • Unreliable reporting — no straightforward way to produce a current picture of client care levels (HCP 1–4)
  • Communication gaps — important information missed because it was never filed against the right client
  • Rostering friction — juggling internal staff, external contractors and medical contacts across recurring visits

Why this is worse in care than in most industries

In a lot of businesses, disorganised admin costs money. In community care it also costs continuity: a case manager picking up an unfamiliar client needs to know what has actually happened, and a note sitting unfiled in someone's inbox is effectively lost.

Aged care providers also carry record-keeping obligations around Aged Care IDs, HCP levels and MMM ratings. Records scattered across spreadsheets make regulatory visibility genuinely difficult rather than merely inconvenient.

The solution

How the platform works

The design goal was an invisible administrator — AI doing the filing, summarising and retrieval in the background so staff spend their time on service delivery rather than data entry.

    Communications arrive as normal

    Email and SMS come in through Mailgun and Twilio integrations. Staff and families don't change how they communicate — no new app to adopt, which is usually where care-sector rollouts fail.

    AI identifies the client and files the message

    AI flows analyse the content, match it to the correct client record, and write it in as a structured case note. The filing step that used to be manual simply stops existing.

    Case notes become a narrative

    AI summarisation generates a concise account of a client's entire interaction history, so a case manager can get up to speed in seconds instead of reading months of individual entries.

    Staff ask questions in plain English

    A dashboard AI assistant answers natural-language queries against the live database, for example:

    • “Show me the last three notes for this client”
    • “Who is assigned to the gardening task on Tuesday?”
    • “Which clients haven't had contact in a fortnight?”

    Rosters handle real-world complexity

    A purpose-built weekly calendar manages support worker schedules including fortnightly and weekly recurrence, plus substitute worker assignment when someone calls in sick.

    Medical specialties, contractor types and client care levels (HCP, MMM, Aged Care ID) are all managed through the interface. Adding a new category doesn't require a developer.

Design principle

Automate the filing, not the judgement

AI does the clerical work here — matching a message to a client, structuring it as a note, summarising a history. It does not decide what care someone needs.

That boundary matters more in this sector than most. Care decisions carry consequences for vulnerable people, and a summary is a starting point for a professional, not a substitute for reading the record when something is at stake.

It's the same principle behind every AI system we build: remove the administration, leave the decision with the qualified human.

Outcome

ClientCenter replaced a spreadsheet-and-inbox operating model with a single system where the record keeps itself up to date. The provider reports an 80% reduction in manual filing, with communications now captured automatically rather than transcribed by staff.

Beyond the time saved, centralising Aged Care IDs, HCP levels and MMM ratings gives the provider regulatory visibility that simply wasn't achievable across scattered files — and lets case managers spend their attention on service delivery instead of data entry.
